What Is All-on-4®?

The name describes the method: a complete arch of fixed teeth carried on four dental implants. The protocol was developed by Professor Paulo Malo with Nobel Biocare, the Swiss implant manufacturer, and has been in clinical use since 1998.

Rather than placing a separate implant for every missing tooth, two implants are positioned at the front of the jaw and two towards the back at roughly a 45-degree angle. Angling the rear implants allows us to anchor into the bone that is available while working around the nerves and sinuses — which is often why treatment remains possible for people who have been told elsewhere that they need extensive bone grafting.

Your new teeth are attached within 24 hours of surgery, so you leave with a complete smile rather than facing a healing period without teeth.

Not every case suits the standard four-implant configuration. Our dentists also work with All-on-6 full arch implants and with longer pterygoid or zygomatic implants where bone loss is more advanced, and will recommend the approach that fits your anatomy and treatment goals.

Am I Suitable?

Full arch dental implants, including All-on-4®, suit many healthy adults — but not everyone.

Treatment is usually considered where you are missing most or all of the teeth in an arch, where your remaining teeth are failing, or where you are struggling with a loose lower denture. Reduced jawbone volume is not necessarily a barrier on its own: the angled implants are designed to use the bone you already have, and longer pterygoid or zygomatic implants extend what is possible further again.

Things that need closer assessment include untreated gum disease, smoking, heavy teeth grinding, and some general health conditions or medications that affect healing and bone. None of these rules you out automatically, but each one shapes the plan — and occasionally means implants are not the right answer.

Our Process

1. Consultation and 3D Smile Planning

Your first appointment establishes two things: what you want from treatment, and whether your jaw can support it. We carry out a full examination alongside 3D diagnostics and facial scanning to assess your bone volume and confirm your suitability, then set out a personalised plan with the costs in writing.

If you decide to go ahead, we take your pre-surgical records and introduce you to our laboratory technician, who works with you on the shape and shade of your new teeth so they suit your face rather than a standard mould.

2. Implant Placement & Immediate Teeth

On the day of surgery your dentist removes any remaining teeth and places between four and six implants into your jawbone, positioned to make the most of the bone you have. Where your plan includes bone grafting or pterygoid implants, this is carried out in the same appointment.

The gums are then closed and digital impressions taken to record exactly where your implants sit, so our in-house technician can design your bridge to match. Within 24 hours you return to have your custom acrylic bridge fitted, made in our onsite laboratory from durable, high-quality acrylic.

Having the laboratory on site keeps the quality, fit and aesthetics of your bridge under our control throughout, and means adjustments can be made without sending work away.

3. Healing & Guided Reviews

Over the following 3–6 months your implants fuse with the surrounding bone through a natural process called osseointegration. This is what gives your final teeth their stability, and it is not a stage that can be rushed.

You will come in for a series of scheduled reviews across this period so we can monitor how you are healing, check the stability of each implant, and make sure your temporary teeth stay comfortable and functional. We will also take you through caring for them day to day, including how to clean around your bridge using the water flosser provided with your treatment.

4. Crafting Your Long-term Teeth

The acrylic bridge fitted within 24 hours of surgery is a working set of teeth rather than a placeholder — you leave with a functional, natural-looking smile while your implants integrate. Cared for properly it can last up to two years, though it is designed for the healing phase rather than the long term.

When you are ready to move on, we take precise digital scans and work with our laboratory technician to design your zirconia bridge. We refine its shape, shade and fit against your facial features, smile line and bite until the result looks balanced and behaves naturally.

5. Ongoing Care & Maintenance

Full arch implants need looking after, and that continues for as long as you have them. You will attend maintenance appointments every 6 to 12 months, with x-rays every 12 to 24 months so we can monitor the health of the jawbone beneath your bridge.

These visits let us clean the areas you cannot reach at home, pick up wear or minor chips early while they are still simple to repair, and support the long-term health of your mouth.

When to Replace Your Bridge

Your temporary acrylic bridge carries a 12-month warranty. You can move to a zirconia bridge at any point from three months after surgery, or keep wearing the acrylic one for up to two years provided it is well looked after.

The Revive Essentials and Revive Advanced packages already include a replacement ceramic bridge and carry five-year warranties. Zirconia teeth commonly up to 18 years or more, though how long any bridge lasts comes down to your oral hygiene and lifestyle factors — teeth grinding in particular*.

Minor chips and everyday wear are normal over time. Where it is appropriate to do so, we repair these in our onsite laboratory, often on the same day, rather than replacing the whole bridge.

Full Arch Dental Implants (All-on-4®) Reviews

You will not find patient reviews of All-on-4® treatment on this page, and that is deliberate.
